Neither. Whenever you see a size associated with a fitting, it's telling you what the fitting pairs with. But you must also look at the end type to see what it pairs with.

A spigot end type will have the same OD and ID as pipe. It will fit inside of a socket end fitting of the same size.
A socket end type will fit OVER pipe/pipe will fit INSIDE of the it.
Obviously, threads go with threads (male thread to female thread and vice versa).

The 2" end of this coupling is socket, so it will fit OVER pipe of 2".
The 1-1/4" end of this coupling is also socket, so it will fit OVER pipe of 1-1/4".
